We report the anesthetic management of a 90 year old patient who was referred by his primary physician for ENT evaluation of progressive dyspnea, worsening over a period of 4 months. The patient was a heavy smoker, and gave a history of binge drinking. Significant past medical history included coronary artery disease and hypertension. The electrocardiogram showed left axis deviation and borderline criteria for left ventricular hypertrophy. The echocardiography confirmed significant LV diastolic dysfunction and aortic sclerosis. Laboratory tests were within normal range.
